Insurance

Warren Central High School and Warren Ultimate Club DOES NOT PROVIDE insurance coverage for student-athletes.  All student-athletes or parents/guardians may purchase supplemental insurance at their own expense through plans found at www.studentinsurance-kk.com.  Warren Ultimate Club coaches are safety conscious; however, due to the nature of athletic activity, injury may still occur.  While the Club provides opportunity for such participation, the parent/legal guardian retains the right of denial of such participation and must carry the responsibility of providing medical care and insurance coverage for their son or daughter.  Parents/legal guardians are encouraged to have a family insurance policy to cover athletic injuries and the cost of treatment.  It should be noted that neither Warren Central High School or Warren Ultimate Club carry any kind of first dollar medical insurance for athletic injuries.

Student-Athlete Commitment

Classroom
I am a student first and an athlete second.  I will keep my attendance excellent, homework current, and grades up.
Character
I will exemplify high standards of integrity, on and off the field.  I will not employ illegal tactics to gain an undeserved advantage.  I will avoid the use of profanity and activity that harms others or their property.  I will tell the truth.  I will not partake in any degree or be in possession of alcoholic beverages, tobacco products, or other inhaled vapor products.  I will not knowingly possess, use, transmit or be under the influence of any opioid, narcotic or hallucinogenic drug, stimulant, depressant, marijuana, counterfeit caffeine pills, anabolic steroids or other performance enhancing drug, or use or transmit paraphernalia for use of such substances.  Use of an authorized drug as presribed by a registered physician is not a violation of this rule.  Student athletes are expected to leave social events immediately where there is illegal use of chemical substances, alcohol or tobacco, or other inhaled vapor products.
Community
I will serve my community through service projects and be a positive influence in all I attempt to do, being helpful always.  I will not engage in illegal conduct of any kind, particularly conduct classified as a felony, midemeanor, or act of delinquency.  I will not be a part of hazing or bullying of others on this team or at school.  I will not embarrass my school or team.  I will have a "Most Valuable Teammate" mentality at all times, and care for my coaches and teammates, treating them as I would like to be treated.
Competition
I will give my best energy and effort every time I play, practice, or represent Warren.  The good of the team is first and foremost. I will follow the specific training and practice rules of Warren Ultimate Club as given by coaching staff.  I will be at every practice, game, or event that I can, and will communicate to the staff when I have a conflict.  I will be on time.  I will respond positively to coaching.  I will take pride in all I do.

Student/Parent/Guardian Certification of Knowledge and Release 

I/we acknowledge that we have reviewed this copy of the 2019-2020 Warren Ultimate Club handbook which includes the Student-Athlete Commitment and Code of Conduct.  I/we understand the code applies to the said student year-round.  Said student-athlete agrees to participate and conduct herself or himself in accordance with the rules of the code and any other specific rules of the coaches or coordinators.  I/we know that athletic participation is a privilege.  The student athlete voluntarily accepts any and all responsibility for her/his own welfare while participating in athletics, with full understanding of the risks involved.  Additionally, I/we grant the club and its representatives the right to take photographs and/or video of my student-athlete and my property in connection with club events.  I/we authorize the club, its assigns and transferees to copyright, use and publish the same in print and/or electronically.  I agree that the club my use such photographs of me with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and web content.  I/we understand that travel is necessary and accident causing injury is a possibility.  I give permission for my student-athlete to be transported to and from competitions or practices in vehicles of volunteer drivers as needed.  With full understanding of the risks involved, I/we release and hold harmless Warren Central High School and other schools, the coaches, coordinators, parents, and sponsors involved of any and all responsibility and liability for any injury or claim resulting from such athletic participation, and agree to take no legal action against the aforementioned because of any accident or mishap involving the athletic participation of the student-athlete.  I understand Warren Central HS and Warren Ultimate Club DOES NOT offer student-athlete insurance.

Due to the new law "Student Athletes: Concussions and Head Injuries" (IC 20-34-7), schools are now required to distribute information sheets to inform and educate student-athletes and their parents of the nature and risk of concussion and head injury to stud-athletes, including the risks of continuing to play after concussion or head injury.   The law requires that each year, before beginning practice for for an interscholastic or intramural sport, a HS student-athlete and the student-athlete's parents must be given an information sheet, and both must sign and return a form acknowledging receipt of the information to the student-athlete's coach.  The law further states that a high school athlete who is suspected of sustaining a concussion or head injury in a practice or game, shall be removed from play at the time of injury and may not return to play until the student athlete has received a written clearance from a licensed health care provider trained in the evaluation and management of concussions and head injuries.
Parent/guardian, please read the above forms "Heads Up--Concussion in HS Sports--A Fact Sheet for Parents" and ensure your student has also read the form "Heads Up--Concussion in HS Sports--A Fact Sheet for Athletes."  After reading these fact sheets, please sign below and ensure your student-athlete also signs the form.
